Donating your old cell phones is easy!
Just follow the steps below to receive a pre-paid mailing label. Then follow the simple shipping instructions.

Step 1:
Choose the state and organization that will receive the proceeds from your donation.


Step 2: Choose the number of cell phones you are donating. Less than 4 - After submitting this form you will be redirected to your pre-paid mailing label. You can then click the "Print" button to print your label. Shipping instruction will be included on the printed page and are listed below. 4 or More - When shipping 4 or more cell phones, you will be required to use a pre-paid FedEx shipping label. After submitting this form you will be redirected to the FedEx Label Creation form where you will enter your information. When this form is submitted, a pre-paid FedEx shipping label will be created for you. This shipping label will be sent to you, in Adobe PDF format, via e-mail at the address you provide. Detailed shipping instructions will be included in the e-mail and are listed below. Number of cell phones to donate:
Step 3: After you have selected your desired state, organization, and number of cell phones to donate, click the Submit button.

Shipping Instructions

Less than 4 cell phones:
When you donate less than 4 cell phones, you will be given a pre-paid Business Reply Label. Follow the steps below to ship your donation to our recycling center:

Step 1: Keep battery attached to phone. If battery is not attached to phone, place tape over terminal ends.

Step 2: Print out your pre-paid mailing label and cut it out along the dashed line.

Step 3: Place cell phones in a small envelope or box.

Step 4: Affix your mailing label to the package.

Step 5: Seal the package and drop it in the mail. Postage is paid!

4 or more cell phones:
When you donate 4 or more cell phones, you will be given a pre-paid FedEx Label. Follow the steps below to ship your donation to our recycling center:

Step 1: Keep battery attached to phone. If battery is not attached to phone, place tape over terminal ends.

Step 2: Print your pre-paid shipping label. Your pre-paid shipping label will be attached to the email you receive after filling out the FedEx Label Creation form. The label will be in PDF format. If you do not have Adobe PDF Reader, Click here. Open the PDF document and print the contents. There may be multiple PDF attachments depending on the amount of handsets you are sending. Be sure to print all of the attachments and affix ONE label to each box.

Step 3: Pack your boxes. Count the number of cell phone handsets and keep for your records. Please use reasonably sized boxes to avoid heavy and cumbersome handling. We suggest office paper boxes or similar sized boxes. Place the phones flat across the bottom of the box and layer them with newspaper or bubble wrap to avoid damage during shipping. Phones and accessories can be shipped in the same box. If you have several boxes, however, you might consider separating them to make processing faster. Seal the boxes securely with packing tape.

Step 4: Label your boxes. Attach your pre-paid FedEx Ground labels that are included in the email, to the boxes. Please make note of the Reference Number(s) from the FedEx label(s) and keep the tracking information along with the number of phones you shipped. Indicate the number of boxes in your shipment (e.g. 1 of 3, 2 of 3, etc.) on each of the boxes. This will ensure our recycling center receives all of your boxes.

Step 5: Ship your boxes. For your convenience, there are 3 ways to ship your FedEx labeled boxes.
a. You can give your boxes to a FedEx courier.
b. Call 1.800.GO.FEDEX (1.800.463.3339) to arrange a Ground pick up at your convenience. Your account number is 2868-4588-6.
c. Take your boxes to a local FedEx retail location (Such as Kinko's).

You are transferring ownership of the enclosed cell phones, batteries and accessories directly to a recycling center. The recycling center will resell or recycle them. Qwest Corporation has no right, title, liability or obligation for them. You release Qwest Corporation (and its officers, members, directors, shareholders, affiliates, employees, agents and representatives) from all claims, losses or liabilities for the enclosed devices. These cell phones, batteries and accessories are not tax deductible.
